Baker-72, Julian PR-1 variety, struck at the U.S. Mint circa 1859.  One of just 57 struck in silver from these dies, though this is the only example we’ve seen with a loop for suspension.

The piece itself is a pretty silvery blue with much original flash remaining.

Housed in a neat NGC slab expertly modified by them to accommodate the loop.
